Who Is Eligible?
HealthPath Foundation of Ohio provides grants to nonprofits and government organizations in our 36-county service area. The specific guidelines for each grant program may vary. See the specific grant program for details.
What is the Process?
Grants are awarded on a competitive basis. Specific grant guidelines are tailored to each initiative and are explained in each request for proposals.
Current Grant Opportunities:
In 2021, HealthPath initiated a shift to multi-year funding and was a direct result of hearing feedback from our partners over the past several years. This funding initiative replaced the separate, one-year funding RFPs for both workforce development and direct service. Although HealthPath will provide fewer grants overall, we believe (along with partners) that these investments will help produce outcomes for communities across Ohio. Opportunities to receive multi-year funding will be available again in 2024.
HealthPath’s other grant opportunity is through Community Connections. Each grant award is in the amount of $8,000. Applications for Community Connections will be available again in 2025.
