The 2025 Community Connections grant cycle commemorated HealthPath’s 25th anniversary by inviting organizations to apply for grants of up to $25,000 to support initiatives in at least one of HealthPath’s areas of impact that advance health equity by ensuring Ohioans have access to essential resources for optimal health. By offering flexible general operating support grants, HealthPath aims to enable organizations to allocate resources where they are most needed, fostering innovation, impact, and sustainable systems change that promotes health equity in Ohio's communities.
