REQUESTS FOR PROPOSALS
Each RFP we issue provides details on funding priorities, anticipated outcomes, and the review process. Funding is awarded to applicants whose proposals best align with HealthPath's mission and vision while meeting the specified RFP requirements.

INVITATION
At times, we may extend funding invitations to organizations. These invitations to apply are based on clear alignment with HealthPath’s areas of impact and often are a result of an emergent need in the community. These funding opportunities are identified through community engagement.

HealthPath’s funding is intentionally flexible. While our focus areas are broad, we are committed to resourcing opportunities that sit at the intersection of priority populations, core focus areas, and impact levers. This enables our funding decisions to align with community-defined needs while maintaining flexibility to refine priorities on an annual or grant cycle basis.

FUNDING ELIGIBILITY

Organizations must also be based in and serving HealthPath’s 36-county service area.
HealthPath defines “based in” as having the organization’s official, primary address located within its 36-county service area. Organizations outside of this area are generally not eligible for funding unless explicitly stated in specific funding opportunities or RFPs.

HealthPath awards grants to 501(c)(3) nonprofit organizations and governmental agencies. Community-based organizations without nonprofit tax status may be able to apply through fiscal sponsorship.
