2021 Community Connections Grants

Grants Will Achieve Healthy Ohioans, Oral Health and Safe Elders


CINCINNATI, Ohio –

The HealthPath Foundation of Ohio (HealthPath) recently awarded $184,000 in grants to 23 organizations dedicated to improving community health across Ohio. The grants are part of HealthPath’s Community Connections initiative and offer the grant recipients general operating support of $8,000 over one year.


“Our Community Connections initiative helps lay the groundwork for bringing community partners together to improve the health of all Ohioans,” said Eric DeWald, president of HealthPath. “Changes in health and healthy behaviors rely on solutions built upon the strengths and needs of the people living in the community. We are proud to partner with these exemplary nonprofit organizations to help all Ohioans reach their fullest health potential.”

The 2021 Community Connections initiative focuses on organizations that have strong community engagement strategies and achieve one of HealthPath’s results areas of Oral Health to ensure Ohioans at every stage of life have access to oral health education and services; Healthy Ohioans to ensure residents have access to the services they need to be healthy, including access to care and healthy food; and Age-Friendly Communities to ensure that older adults live and participate in communities to their fullest potential, free from abuse, neglect and exploitation.

• Abuse & Rape Crisis Shelter of Warren County (serving Warren County)

• Brigid's Path (serving Auglaize, Butler, Carroll, Clark, Clermont, Clinton, Darke, Greene,

Hamilton, Highland, Mercer, Miami, Montgomery, Preble, Shelby, Warren Counties)

• Child Care Choices, Inc. (serving Clark, Miami, Montgomery, Shelby Counties)

• Diabetes Youth Services (serving Allen, Auglaize, Hancock, Hardin, Mercer, Putnam, Van

Wert Counties)

• Epilepsy Alliance Ohio (serving multiple counties in HealthPath’s service area)

• Health Care Access Now (serving Butler, Clermont, Hamilton, Warren Counties)

• Health Partners Free Clinic (serving Miami County)

• HealthSource of Ohio (serving Adams, Brown, Clermont, Clinton, Hamilton, Highland,

Warren Counties)

• Lima Symphony Orchestra (serving Allen, Auglaize, Hancock, Hardin, Mercer, Putnam,

Van Wert Counties)

• National Society to Prevent Blindness, Ohio Affiliate (statewide service)

• New Choices Inc. (serving Shelby County)

• North American Indian Cultural Center, Inc. (serving Columbiana, Holmes, Mahoning,

Stark, Tuscarawas Counties)

• Ohio Domestic Violence Network (statewide service)

• Ohio Valley Health Center (serving Harrison and Jefferson Counties)

• Pink Ribbon Girls (serving Adams, Auglaize, Brown, Butler, Clark, Clermont, Clinton,

Darke, Greene, Hamilton, Highland, Mercer, Miami, Monroe, Montgomery, Preble,

Shelby, Stark, Warren Counties)

• Sidney-Shelby County YMCA (serving Shelby County)

• Talbert House (serving Hamilton County)

• Youngstown Lifeguard Academy (serving Mahoning County)

• Area Agency on Aging 3 (serving Allen, Auglaize, Hancock, Hardin, Mercer, Putnam, Van

Wert Counties)

• Episcopal Retirement Services (serving Clark, Clermont, Clinton, Hamilton, Montgomery

Counties)

• Family Violence Prevention Center of Greene County (serving Greene County)


The 2021 grantees creating Oral Health includes:

• Community Action Committee of Pike County (serving Adams and Scioto Counties)

• Good Neighbor House (serving Clark, Darke, Greene, Hamilton, Miami, Montgomery,

Preble Counties)


Since the Community Connections program began in 2012, HealthPath has awarded over $1.9 million through 241 grants to organizations throughout Ohio.
